Iowa Medicaid for a Grandchild in Kinship Care

You Do Not Need Guardianship to Get Medicaid for the Child

This is the fact that matters most and the one caseworkers do not always volunteer: a relative caregiver in Iowa does not need legal guardianship, custody, or any court order to apply for Medicaid or Hawki on behalf of a child living in their home.

If the child lives with you, you can file the application. Period.

Iowa children may qualify for Medicaid or Hawki (Healthy and Well Kids in Iowa, the state's CHIP program) up to 307% of the Federal Poverty Level under the applicable child and household rules.

How the Application Works

Apply through the Iowa HHS Benefits Portal online or with a paper application at your local county HHS office. The application covers Medicaid, Hawki, and FIP simultaneously — you check which programs you are requesting.

You will need the child's Social Security number and proof of citizenship. If the parents left without the child's documents and you cannot locate the Social Security card, Iowa HHS is required to assist with electronic verification through the Federal Data Services Hub rather than denying the application outright.

Processing time is 45 calendar days (90 days if a disability evaluation is involved).

Retroactive Coverage and Continuous Eligibility

Two provisions protect kinship families from medical debt:

Retroactive coverage. Medicaid can cover up to three months of medical expenses incurred before the application month, as long as the child would have been eligible during that period. If your grandchild needed an ER visit or prescription the month before you applied, those costs can be covered retroactively.

12-month continuous eligibility. Once approved, the child's coverage continues for a full year regardless of minor income fluctuations in the household. A small raise or a one-time insurance payout will not trigger a mid-year coverage loss.

Coverage starts on the first day of the month you apply — not the date the approval letter arrives.

What Medicaid Covers

Iowa delivers Medicaid through managed care organizations under Iowa Health Link. The child's benefits include:

  • Preventive well-child visits and immunizations
  • Outpatient therapy and specialist referrals
  • Integrated dental and vision services
  • Behavioral health crisis support and counseling
  • Prescription medications
  • Non-Emergency Medical Transportation (NEMT) to scheduled appointments

That last one is easy to miss. If you do not have reliable transportation to a pediatrician 40 miles away in a rural county, NEMT can cover the ride.

Children in Foster Care or Guardianship Subsidies

Children placed in court-ordered foster care or receiving a Title IV-E guardianship subsidy automatically qualify for categorical Medicaid — no income test at all. For IV-E-eligible children receiving subsidized guardianship, Medicaid coverage transfers across state lines through the Interstate Compact for Adoption and Medical Assistance (ICAMA).

When to Appeal a Denial

If Iowa HHS denies a Medicaid application or terminates coverage, you have 90 calendar days from the notice date to request an administrative fair hearing. To keep coverage active while the appeal is pending, request the hearing within 10 calendar days of the notice date or before the effective date of the adverse action.

Service denials — such as a managed care organization refusing to authorize a specialist visit — follow a different process. Appeal internally with the MCO first, then escalate to a state fair hearing if the MCO upholds the denial.

Medicaid and Other Benefits

Medicaid, FIP child-only, and SNAP use separate eligibility rules and income calculations. Children receiving Medicaid or FIP are adjunctively eligible for WIC (if under 5). Children receiving FIP or SNAP are directly certified for free school meals.
