Hawaii Caregiver Consent by Island: Oahu, Maui, Big Island and Kauai
Same Law, Different Logistics
Hawaii's caregiver consent statutes — HRS § 577-28 for medical care and HRS § 302A-482 for school enrollment — apply identically on every island. The medical affidavit statute excludes children placed under DHS custody under chapter 587A. A notarized affidavit executed on Kauai carries the same legal weight as one from Oahu. What differs island to island is the practical logistics: where to find notary services, which school complexes handle enrollment, and which Legal Aid office serves your area.
Oahu
Oahu has the most resources for kinship caregivers, but also the most bureaucratic layers.
Notary services: banks, UPS stores, law offices, and some community centers offer notarization. FedEx Office locations in Honolulu and Pearl City offer online notary assistance, but not in-store notarization; they can help print and scan documents after an online session. HRS § 456-17 lists a $5 fee per signer for an in-person acknowledgment and $5 for administering an oath.
School enrollment: HIDOE operates multiple complex areas on Oahu. Bring the completed Educational Caregiver Affidavit (HRS § 302A-482), proof of your residential address, the child's birth certificate, Form 14, immunization records, and TB Clearance Form F to the school serving your attendance zone.
Legal Aid: the main Oahu office (808-536-4302) is your first call if you hit resistance at a school or clinic. They can verify your affidavit, provide templates, and offer legal guidance for complicated situations.
Medical care: community health centers like Waikiki Health and Kalihi-Palama Health Center are familiar with caregiver consent. Bring your notarized HRS § 577-28 affidavit to every appointment.
Maui
Notary services: fewer walk-in options than Oahu, but banks and shipping stores in Kahului and Lahaina offer notarization. Call ahead to confirm availability and hours.
School enrollment: contact the school directly in your attendance zone. The SASA (School Administrative Services Assistant) or Vice Principal handles enrollment paperwork. If you encounter resistance, reference HRS § 302A-482 and ask for the Complex Area Superintendent.
Legal Aid: Maui office at 808-244-3731. They serve caregivers across Maui County, including Molokai and Lanai.

Hawaii Island (Big Island)
Notary services: Hilo and Kona both have banks and shipping stores with notary services. In more rural areas like Pahoa, Waimea, or Ka'u, call your bank branch first — they often notarize for account holders at no charge.
School enrollment: the Big Island's school complexes are spread across large geographic areas. The process is identical to Oahu — the same affidavit, the same documents — but travel distances to the school office may be longer. Some schools accept mailed or scanned documents for initial review; call ahead to ask.
Legal Aid: Hilo office (808-961-2851) and Kona office (808-329-8331) cover both sides of the island.
Kauai
Notary services: limited options. Banks in Lihue are the most reliable. Some attorneys' offices will notarize documents for a small fee even if you're not a client.
School enrollment: Kauai's schools are generally smaller and more community-oriented. Enrollment staff may be more familiar with kinship care situations — or less. Either way, bring the complete document packet and the statutory reference.
Legal Aid: Kauai office at 808-245-4728.
Molokai and Lanai
Both islands have dedicated Legal Aid phone lines (Molokai: 808-553-3251; Lanai: 808-565-6089). Notary services are limited — your bank is likely the best option. School enrollment follows the same statewide process.
The Documents Don't Change
Across the islands, HIDOE's current enrollment FAQ says power-of-attorney paperwork is required if a child is not living with their parents, while HRS § 302A-482 separately provides for a DOE caregiver affidavit. Whether that affidavit alone satisfies the current enrollment procedure is unresolved; confirm the school's document requirements.
No matter which island you're on, the core requirement is the same: a notarized caregiver affidavit under the correct statute, plus the standard school or medical intake documents.
