Wisconsin Guardianship Annual Report and Post-Order Duties

The Order Is Just the Beginning

Getting a guardianship order signed feels like crossing a finish line, but it's really the start of a new set of responsibilities. Wisconsin law requires appointed guardians to maintain the order, report to the court, and keep their documentation current. Missing a deadline can create complications with schools, doctors, and benefit agencies.

Here's what's expected of you after the judge signs Form JN-1530.

Get Certified Copies Immediately

Your first task after the hearing: obtain multiple certified copies of your Letters of Guardianship and Dispositional Order (Form JN-1530) from the Clerk of Court or Register in Probate. A certified copy costs $14 plus $1.25 per page through the Clerk of Court, or $10 plus $1.00 per page through the Register in Probate.

Order at least four or five certified copies. You'll need them for the child's school, the pediatrician's office, your health insurance provider, and any benefit agency (county kinship care, BadgerCare Plus, FoodShare). Each institution typically wants to keep a copy on file. Uncertified photocopies are routinely rejected.

Notify Schools and Medical Providers

Within a few days of receiving the order, deliver a certified copy of JN-1530 to the child's school and primary healthcare provider. This ensures you can enroll the child, authorize medical treatment, access school records, and make educational decisions without delays.

If the child needs to change schools, the Letters of Guardianship serve as your enrollment authority. You won't need a separate power of attorney or delegation form once you have the court order.

File Annual Reports

For full guardianships of the person under § 48.9795, the guardian must file an annual report on the child's condition with the appointing court. The Wisconsin Court System provides Form JN-1550; check the order or the court for the report's due date and filing method.

The report typically covers the child's living situation, health status, educational progress, and any significant changes since the last filing. If you hold a consolidated guardianship that includes the estate (under Chapter 54), you must also file a Guardianship Inventory (Form GN-3445) within 60 days of appointment, listing the child's financial assets, and submit annual financial accountings to the Register in Probate.

Check your specific order and ask the Register in Probate in your county about their reporting calendar. Setting a recurring reminder is the simplest way to stay on track.

Apply for Benefits

With your certified Letters in hand, you're eligible to apply for several programs:

  • Kinship Care — the 2026 and 2027 rate is $384 per month per child for caregivers who meet the program's eligibility requirements
  • BadgerCare Plus — if you have court-awarded legal custody and are a qualifying relative, the state treats you as a caretaker relative. If you want both yourself and the child included in one case, you must apply as the caretaker relative; eligibility uses BadgerCare's household rules.
  • FoodShare — additional food assistance through the ACCESS portal

The guardianship order strengthens your application because it proves your legal authority over the child. Detailed eligibility rules and application steps live in a separate guide dedicated to Wisconsin kinship financial help.

Keep Your Order Current

If anything changes — your address, the child's school, a parent's circumstances — note it for your next annual report. Major changes may require a formal modification through Form JN-1540, particularly if you need to adjust the scope of your authority or relocate out of state.

Stay Organized From Day One
