Ohio Annual Guardian Report (Form GM.1): What to File and When

Getting appointed guardian of a grandchild in Ohio Probate Court is not the end of the paperwork — it is the start of an ongoing reporting obligation. Guardians of the person must file the reports required by the Probate Court that appointed them, and guardians of the estate have additional financial accounting requirements on top of that. Form numbers and deadlines can vary by county: Summit County calls its minor-guardian report Form GM.1, while the Supreme Court of Ohio's statewide forms catalog lists a Guardian's Report as Form 17.7. Use the form supplied by your appointing court.

What the Annual Guardian's Report Covers

Summit County's Form GM.1 asks for a summary of the minor's situation and whether the guardianship should continue:

  • Basic information — the child's age, date of birth, current address, who the child lives with, and the school attended
  • School and court issues — serious discipline or truancy problems, and any charges or referrals to Juvenile Court during the reporting period
  • Safety concerns — whether an abuse report was filed or the guardian has information about possible abuse
  • Other concerns — any other problem involving the child or guardianship that should be brought to the court's attention
  • Need to continue — whether the guardianship remains necessary

The report gives the court a snapshot of whether the guardianship continues to serve the child's interests. Answer each question on the court's form accurately, even when nothing changed.

The Annual Guardianship Plan

Whether a minor's guardian must file a separate plan depends on the appointing court's rules. Summit County's minor-guardianship instructions identify Form GM.1 as the annual report; ask your court whether it requires a separate plan and which local form to use. Do not assume the adult-guardianship Form 27.7 applies to a minor.

When to File

Your filing deadline is set by the court that appointed you. Summit County requires Form GM.1 on the anniversary of the guardian's appointment; other counties may use a different form or schedule. Check your Letters of Guardianship or the court's local rules for your exact deadline.

The Probate Court clerk's office may send a reminder, but do not rely on it. Set your own calendar reminder 30 days before the due date to give yourself time to gather information and complete the form.

Estate Guardianship: Additional Financial Reporting

If you were also appointed guardian of the child's estate (managing assets like an inheritance, insurance payout, or settlement), your reporting obligations are substantially heavier:

  • Inventory (Form 15.5) — due within 90 days of your appointment, listing every asset in the child's estate
  • Annual or biennial Accounts (Form 15.8) — detailed financial statements covering every receipt, disbursement, and investment made on behalf of the estate during the accounting period
  • Bond — estate guardians must post a bond (a form of financial insurance) with the court

Every dollar must be documented. The court reviews your accounts to ensure the child's assets are being properly managed and preserved. Receipts, bank statements, and investment records should be organized throughout the year, not scrambled together at filing time.

What Happens If You Miss a Filing

Missing a required report can prompt the Probate Court to follow up and review the guardianship. Do not assume the appointment ends automatically; respond to any court notice by its stated deadline, and contact the clerk if you are unsure what to file. Submit the court's actual form even if nothing changed during the year.

Juvenile Court Legal Custody: Different Rules

If your legal authority comes from a Juvenile Court legal custody order rather than a Probate guardianship, you generally do not have annual probate guardian-report or financial-accounting obligations unless the court specifically ordered them. However, the court retains jurisdiction and can require reports if it chooses.

The Ohio Guardianship and Custody Court Process Guide includes a post-appointment compliance calendar that tracks annual report deadlines, estate accounting dates, and notification requirements so you never miss a filing.
