Idaho School Enrollment Documents and Immunization Requirements

You have a child living with you and need to get them into an Idaho school. Here is exactly what the district office will ask for, what counts, and what to do when documents are missing.

The Document Checklist

Idaho public schools typically require four things at enrollment:

  1. Proof of address — A utility bill, lease agreement, or mortgage statement showing your name and home address within the district boundary. If you live in a family member's home and the bills are not in your name, a signed residency affidavit from the homeowner works in most districts.

  2. Identity and age verification — A certified birth certificate is the standard. Under Idaho Code § 18-4511(2), a person enrolling a student for the first time has 30 days to provide a certified copy or other reliable proof of the student's identity and birth date. If you cannot get a birth certificate in time, a government-issued photo ID, passport, or hospital record with an explanatory affidavit satisfies the requirement.

  3. Immunization records — Idaho requires immunization documentation or an exemption on file for school entry. Children who are in the process of receiving required immunizations may be conditionally enrolled. Idaho allows medical, religious, or other exemptions. A medical exemption must be completed by a licensed physician, physician's representative, or another licensed healthcare professional; religious or other exemptions may be documented on the state form or a signed written statement. The exemption form is available at the school office.

  4. Power of Attorney or custody documentation — If you are not the biological parent, bring a notarized Power of Attorney under Idaho Code § 15-5-104 (the free CAO P2 form from courtselfhelp.idaho.gov works) or a court guardianship order.

The 30-Day Birth Certificate Rule

Idaho law gives families 30 calendar days from enrollment to produce a birth certificate or equivalent. The school enrolls the child immediately — this is not a reason to delay enrollment.

If the person enrolling still has not provided proof after 30 days, the school must notify local law enforcement and give the person 10 additional days to comply. The student's ability to attend is not impacted.

Replacement birth certificates cost $16 per certified copy and must be ordered online or by mail from the Idaho Bureau of Vital Records; there is no public counter. A caregiver with a POA or guardianship order can request one on the child's behalf.

Immunization Specifics

Idaho's minimum school-entry schedule includes DTaP, hepatitis A and B, MMR, polio, and varicella. Seventh-grade entry also requires a Tdap booster and meningococcal vaccine; grade 12 has an age-dependent meningococcal requirement.

If immunization records were at the child's previous school, ask the new school to request them directly. A child who is catching up on required vaccines may be conditionally enrolled; a child eligible under McKinney-Vento must be enrolled immediately even if immunization records are missing.

When Documents Are Missing Entirely

Children in kinship placements often arrive with nothing — no birth certificate, no shot records, no prior school records. For a child eligible under McKinney-Vento, missing records cannot delay immediate enrollment. Idaho also allows conditional enrollment while a child is catching up on required immunizations.

Tell the registrar what you have: if the child is McKinney-Vento eligible, request immediate enrollment; if the child is catching up on required immunizations, request conditional enrollment. Ask for the district McKinney-Vento liaison if any pushback continues.
