Florida District-by-District School Enrollment for Grandparents and Relative Caregivers
Every Florida District Has Its Own Rules
Florida has no statewide caregiver enrollment form for schools. Each of the state's 67 school districts sets its own process for non-parent enrollment, which means the form name, the required documents, and the approval process differ depending on where you live.
Here's what the six largest districts require from grandparents and relative caregivers.
Orange County Public Schools (OCPS)
Orange County is one of the stricter districts. OCPS does not accept informal caregiver notes. If you don't hold legal custody, you need to schedule an appointment with the central Office of Student Enrollment to obtain an official "Educational Guardianship" or "Parent Affidavit" form.
What to bring:
- Your government-issued photo ID (driver's license or state ID)
- Proof of residency in the Orange County school zone (lease, utility bill, or property tax statement)
- The child's birth certificate
- A notarized statement from the custodial parent explaining the arrangement — or, if the parent is unreachable, a written sworn statement from you explaining the circumstances
OCPS processes these centrally, so don't go to the school first. Call the Office of Student Enrollment to schedule your appointment.
Miami-Dade County Public Schools (M-DCPS)
Miami-Dade uses Form FM-7402 (Caregiver's Authorization Form) for non-parent enrollment. The form requires:
- Your government photo ID
- Two proofs of address (lease plus a utility bill is the standard combination)
- Parent signature on the form, or documented verification that the parent is unlocatable due to hardship
For families experiencing housing instability or informal placements, Miami-Dade coordinates non-parent enrollment through Project UP-START, the district's homelessness program. Project UP-START liaisons work directly with caregivers to facilitate school entry under the McKinney-Vento Act when the child meets its eligibility criteria and standard documentation isn't available.
FM-7402 is valid for one school year and must be renewed annually.

Broward County Public Schools
Broward uses its own Caregiver Authorization Form along with an Unaccompanied Youth Form for students who may qualify under McKinney-Vento. Requirements:
- Your photo ID
- Lease or mortgage documentation plus a utility statement
- An affidavit of residence (Broward provides a template)
- Documentation of parent contact attempt
Broward's form grants enrollment and emergency contact authorization for the current school year. It does not transfer legal custody or create any authority outside the school district.
Hillsborough County Public Schools
Hillsborough County handles non-parent enrollment through its Student Assignment office at each school. Grandparents and relatives need:
- Photo ID and two proofs of address in the school's zone
- The child's previous school records (if available)
- A caregiver affidavit or authorization — check with the specific school for the current form name
Hillsborough is one of the districts where staff familiarity with the caregiver enrollment process varies by school. If the school-level registrar is unfamiliar with the non-parent process, ask to speak with the school's social worker or the district's Student Services department.
Polk County Public Schools
Polk County manages enrollment through district enrollment centers rather than individual schools. You'll need to visit the enrollment center for your zone with:
- Your ID and proof of residency
- The child's proof of age (birth certificate or alternative)
- Specific relative caregiver documentation as required by the enrollment center
Polk's centralized process means the enrollment center staff are generally familiar with kinship enrollment situations.
Duval County Public Schools (Jacksonville)
Duval County's enrollment process runs through individual schools with a standard set of documentation requirements. Relative caregivers should bring:
- Photo ID and two proofs of residency
- The child's immunization records (Form DH 680) and physical exam (Form DH 3040)
- A notarized caregiver authorization or affidavit of residency
Duval's Student Assignment office can clarify the specific form required for non-parent enrollment if the school is uncertain.
When the District Says No
If any district refuses to enroll the child:
- Request the refusal in writing. Florida education code requires the district to explain its decision and notify you of the appeal process.
- Ask about McKinney-Vento eligibility. If the child lacks a fixed, regular, and adequate nighttime residence, including a doubled-up arrangement caused by loss of housing, economic hardship, domestic violence, or a similar emergency, the district must enroll the child immediately under federal law — regardless of missing documents.
- Contact the district's Homeless Education Liaison. This person has the authority to override standard documentation requirements for qualifying students.
