Arizona Guardianship of a Minor Forms by County: Maricopa, Pima, Pinal, and More

Why County Matters

Arizona's guardianship law is the same statewide — A.R.S. Title 14, Chapter 5 — but each county's Superior Court packages the forms differently. Maricopa calls its packet "JG1." Pima uses a numbered packet system. Yavapai labels theirs "Packet 60a." The forms inside accomplish the same thing, but searching for the wrong packet name in the wrong county wastes time.

You file in the county where the child lives or is physically present when you start the case.

Maricopa County

Self-Service Center: Maricopa County Superior Court, Law Library Resource Center (superiorcourt.maricopa.gov/llrc/jg_jg1/)

Guardianship of a Minor packet: JG1 (packet file JG1z). Its initial forms include:

  • Probate Information Form (PB12f)
  • Petition for Appointment of Guardian of a Minor (form JG12f)
  • Affidavit of Proposed Guardian under A.R.S. § 14-5106 (form JG11f)
  • Consent of Parent to Guardianship and Waiver of Notice (form JG13f)
  • Consent of Minor over age 14 (form JG14f)

The Order of Appointment and Letters are handled after the hearing.

Annual Report: Maricopa's Guardianship page says reports are no longer required for Title 14 guardians, but its JG10p instructions still direct guardians to file annual report packet JG9. Check the order in your case and confirm with the Juvenile Court.

E-filing: eFileAZ handles subsequent filings. Initial petitions are typically paper-filed at the Clerk's window.

Fee Waiver: Packet GNDW1, which includes form GNDW11f (Application for Deferral or Waiver of Court Fees).
