Arizona AHCCCS and KidsCare Enrollment for a Grandchild in Your Care

The Caregiver Income Exclusion for Medicaid

When a grandparent or relative enrolls a child in AHCCCS or KidsCare, the eligibility rules work in the child's favor. Under MAGI (Modified Adjusted Gross Income) household composition rules, the child's Medicaid household consists only of the child and any minor biological or adoptive siblings living in the same home. The grandparent's income is excluded from the calculation.

This means a grandparent earning $4,000 a month in retirement income isn't counted in the child's MAGI calculation. A working aunt making $80,000 a year? Her income is excluded too. AHCCCS still evaluates the child's countable income and other eligibility requirements.

AHCCCS vs KidsCare: Which One

Arizona offers two health coverage paths for children:

AHCCCS Medicaid covers children from birth through age 18 with countable household income up to 147% of the Federal Poverty Level. The caregiver-income exclusion does not make enrollment automatic; the child must also meet AHCCCS's other eligibility requirements.

KidsCare (CHIP) covers children in households with income between 147% and 230% FPL. It requires monthly premiums based on income tier. The child's countable income determines which program's income range applies.

The caregiver's income is excluded from the child's MAGI calculation, but AHCCCS still determines eligibility using the child's income and the program's other requirements.

How to Apply Through Health-e-Arizona PLUS

The application for AHCCCS and KidsCare runs through the Health-e-Arizona PLUS portal at healthearizonaplus.gov. You can apply online or through a local AHCCCS eligibility office.

What you'll need:

  • The child's birth certificate or other identity document
  • Proof of citizenship or qualified immigration status
  • Any documentation of the child's own income (Social Security survivor benefits, child support received, trust distributions)
  • Your relationship to the child

No mandatory interview is required. AHCCCS applications are processed within 45 calendar days.

If you're already applying for TANF or SNAP through form FAA-0001A, the same application covers AHCCCS. Check the health coverage box and DES routes the medical portion to AHCCCS.

DCS Placements Get Automatic Enrollment

If DCS placed the child in your home through the child welfare system, the child is enrolled in AHCCCS automatically through DCS administrative channels. You don't need to apply separately through HEAplus. The DCS specialist handles enrollment.

Children receiving Guardianship Assistance Program (GAP) subsidies also maintain AHCCCS eligibility automatically for the duration of the subsidy.

Renewals and Continuous Eligibility

Arizona provides 12 months of continuous eligibility for children on AHCCCS Medicaid. Once approved, the child stays covered for a full year regardless of household changes during that period.

At renewal time, AHCCCS attempts an automated "ex parte" review using available data before asking for paperwork. If the automated review can confirm continued eligibility, you won't need to submit anything. If it can't, you'll receive a redetermination notice with a deadline to respond — missing that deadline terminates coverage.

Caretaker Relative Medicaid for the Grandparent

AHCCCS also offers a separate coverage category for low-income adult relative caregivers. If you're a grandparent acting as the primary caretaker of a child under 19 who is enrolled in AHCCCS, and your own income is below the caretaker relative income standard (roughly $1,912/month for a household of two), you may qualify for AHCCCS coverage yourself.

The Arizona kinship financial help guide includes the full income tables for both the child's Medicaid and the caretaker relative category, along with a checklist of what to bring to your AHCCCS application.
